Forms
Forms allow you to collect additional information from attendees during the registration process. Medtribe supports two types of forms: Application Forms for screening attendees, and Booking Forms for collecting information at checkout.
Understanding Form Types

Creating an Application Form
Application forms are used when you need to screen attendees before allowing them to book. This is useful for events with prerequisites, limited places, or specific eligibility criteria.
- Open your event and navigate to Forms
- Click Add Application Form
- Choose to create from scratch or use a template
- Give your form a name (for internal reference only)
- Add your questions using the form builder
- Save the form
Question Types
You can add various question types to your forms:
- Short text - Single line text input
- Long text - Multi-line text area for detailed responses
- Multiple choice - Select one option from a list
- Checkboxes - Select multiple options
- Dropdown - Select from a dropdown menu
- File upload - Allow attendees to upload documents
- Date - Date picker
- Email - Email address input with validation
- Phone - Phone number input
Reviewing Applications
When attendees submit an application:
- You receive a notification
- Go to Forms â View Responses
- Review each application
- Click Approve or Reject
- Approved applicants can proceed to book
- Rejected applicants receive a notification
Creating a Booking Form
Booking forms collect additional information from attendees during checkout. Unlike application forms, they don't require approval.
- Open your event and navigate to Forms
- Click Add Booking Form
- Choose to create from scratch or use a template
- Give your form a name
- Add your questions
- Save the form

Using Templates
Templates save time by allowing you to reuse form structures across multiple events.
Creating a Template
- Go to Templates in the main navigation
- Select Application Forms or Booking Forms
- Click Create Template
- Build your form with questions
- Save the template
Using a Template
When creating a new form for an event:
- Click Add Application Form or Add Booking Form
- Select From Template
- Choose your template
- The form is created with all template questions
- Customise if needed, or use as-is
Managing Templates
From the Templates section, you can:
- Edit - Modify template questions
- Duplicate - Create a copy to modify
- Delete - Remove templates you no longer need
- Transfer - Move templates to your organisation (if using teams)
Viewing Form Responses
To view responses submitted through your forms:
- Open your event
- Navigate to Forms
- Find the form and click View Responses
- See all submissions with response details
- Filter by session or ticket type if needed
Response Statistics
For each form, you can see:
- Total submissions - How many responses received
- For application forms: Pending, approved, and rejected counts
- Response breakdown - Summary of answers for each question

Best Practices
- Keep forms concise - Only ask for information you need. Long forms reduce completion rates.
- Use templates - Create templates for common scenarios to save time.
- Mark essential questions as required - Use the required flag sparingly for truly essential information.
- Provide clear instructions - Use question descriptions to guide attendees.
- Review applications promptly - Applicants are waiting to book. Quick reviews improve their experience.
- Test your forms - Preview forms before publishing to check the attendee experience.
Troubleshooting
Can't edit a form?
- Forms with submissions cannot be edited
- Duplicate the form and make changes to the copy
Form not showing to attendees?
- Verify the form is attached to the correct event
- Check the event is published
- For application forms, ensure the session requires applications
Template limit reached?
- Delete unused templates
- Upgrade your plan for more templates
